Submitting a FOIA Request

This page is NOT for Police Department FOIA requests. Police requests include, but are not limited to: body-worn camera footage, crash reports, incident/police reports, etc. To make a FOIA request for the Fairview Heights PD, use their form HERE.

If you want to make a Freedom of Information Act request for the City that is not related to the Police Department, please continue reading for instructions. If you are unsure whether your request is PD-related or not, please contact the City Clerk’s Office at 618-489-2000.

Overview

It is the policy of the City of Fairview Heights to fill all requests for public documents in accordance with State law. The majority of requests will be processed within 5 business days. If requests require research or are unusually large, the City may request additional time in accordance with the Act. Requests of a commercial nature are subject to different regulations which extend the response time permitted by law.

Submitting a Request

It is the policy of the City of Fairview Heights to ensure that all requests are properly and promptly completed. The City FOIA Request Form is provided for your convenience. 

Again, do NOT submit Police Department FOIA requests with the City’s form. They will be rejected. For PD requests, use the link to their form at the top of the page.

Fees
By submitting this Request Form, you are agreeing to pay to the City the copying and certification fees beyond the first 50 pages, which are free of charge. After the first 50 pages, the fee is 15 cents per page. Additional charges may be imposed for large format, certain electronic media or color copies, if applicable. You may also request to view the documents in the office. You may refer to the City Chart for a list of departments and other information.
Receiving Your Document

The City will disclose the public records requested on this Request Form within 21 business days after the receipt of this Request Form for all requests made for commercial purposes, and within 5 business days for all other requests, unless the applicable response period is extended as provided by law or the request is denied. All extensions and denials will be in writing and will state the reasons therefore. The requester may seek review of a denial by the Public Access Counselor of the Office of the Illinois Attorney General. Judicial review is available under Section 11 of the Illinois Freedom of Information Act, 5 ILCS 140/1 et seq.
