Apply for a Liquor License

If you are applying for a Liquor License for the first time in the City of Fairview Heights, please complete and submit the Liquor License Application (PDF). If you wish to renew your current Liquor License, please complete and submit the Liquor License Renewal Application (PDF).  Managers have to get a background check and fill out a Manager Application (PDF).  The cost is $28.25.
